A Supplemental Letter is an additional letter, video or tape that you may leave for the important people in your life and for those who will be making decisions for you after your death. There is no right or wrong way to write or create a supplemental letter. It is a way to express your personal thoughts and feelings. You may not be able to say what to do, but you could say something that will help them make difficult decisions about your healthcare and End of Life management. Use the supplemental letter to let your loved ones know more about you, how you think and what your wishes for them may be.
- Do you want to write a supplemental letter in addition to an Advance Healthcare Directive? Why or why not?
- To whom would you write your letter?
- If you write or create a supplement letter, how will you make sure the recipient(s) receives it upon your death?
